Furosemide<br />
It is a very potent drug used orally and intravenously to treat severe CHF and oedema in lungs due to Left Heart Failure. Usual dose is 40 to 80 mg per day. Drug causes loss of electrolytes, particularly potassium. Therefore, it is necessary to get extra potassium (as potassium chloride) during its regular use. One way to obv<span id="more-42"></span>iate loss of potassium is by giving another variety of drug along with furosemide. These drugs are called as Potassium-Sparing diuretics. These counteract the loss of potassium. Such drugs are:<br />
Spironolactone is a combination of furosemide 20 mg and spironolactone 50 mg.<br />
Triamterene (Frusemene) is a combination of furosemide 20 mg and triamterene 50 mg.<br />
Torasemide (Dytor 10 mg). It is another potent diuretic.<br />
Thiazides<br />
For mild to moderate CHF, thiazides act as good diuretics allowing loss of sodium, potassium and water in urine. The same problem exists with their use and therefore potassium supplementation is necessary. Addition of a potassium-sparing diuretic along with a thiazide is quite common practice to reduce potassium loss for example ALDACTIDE which is a combination of hydroflumethiozide 25 mg and spironolactone 25 mg. BIDURET which is a combination of hydrochlorothiazide 50 mg and amiloride 5 mg. In some cases longer use of thiazide may cause impotency, poor control of diabetes, rise in blood levels of uric acid (gouty pains may occur) and lipids. Allergy may occur, occasionally.<br />
Drugs that Provide Long Term Benefits In CHF<br />
ACE-inhibitors<br />
Captopril, (Capoten) Enalapril (Enace), Lisinopril (Listril), Ramipril (Ramace) are the drugs which decrease formation of chemicals aldosterone and angiotensin-II which are responsible for causing structural changes in the heart with CHF. Therefore, their use in low doses for a longer period alters the course of disease (unfortunately digoxin could not do so!). The size and thickness of heart muscles revert back to normal and pumping capacity improves without significant adverse effects. This has been proved in multicentric drug trails which show prolongation of a &#8216;good quality&#8217; life in CHF. The chances of sudden death are reduced and episodes of CHF recurrences are less. Obviously, they are increasingly used nowadays. Patients who are unable tolerate ACE-inhibitors due to dry hacking cough, losartan, candesartan or valsartan is given.<br />
Role of Beta-blockers: It is a parodox that a drug which worsens CHF has shown to be beneficial in long term for the same disorder, provided CHF is treated beforehand with the above drug in the absence of breathlessness and very low doses of metoprolol, carvedilol and bisoprolol are used. After 2 to 3 months the pumping capacity of heart increases and episodes of hospitalisation reduce. The only caveat is that use should be judicious and systolic dysfunction (poor heart contractions) is the underlying problems and the patient has mild to moderate CHF treated or compensated with routine drugs only.<br />
Role of Other Drugs: Spironolactone acts as a mild diuretic but more importantly prevents heart muscle damage (fibrotic changes) when added to ACE-inhibitors in low doses of 12.5 to 25 mg per day. Its benefits are slow but steady. It should never be taken if there is kidney damage. In poor relaxation (diastolic phase) of heart, calcium channel blockers are effective.</p>

This drug is actually a combination of a diuretic, or water pill, and an ACE (Angiotensin Converting Enzyme) inhibitor. Usually it will not be prescribed until the rehabilitation process has begun. Due to the potential fatal risk to the unborn child, pregnant women should never take Zestoretic. Following some simple rules will not only reduce your chance of having a problem, but should reduce your cost as well.</p>
<p>1. Always shop around for medications  there is a signific<span id="more-40"></span>ant difference in cost from pharmacy to pharmacy. Check both online and offline sources. For example, several of my patients have realized a significant cost savings by using the Costco pharmacy as well as online Canadian pharmacies. Should you choose to use a Canadian pharmacy, please keep abreast of any laws that may affect your purchase.</p>
<p>2. Always check to see if a generic equivalent is available. Today, there are very few medications on the market where generics are not acceptable.</p>
<p>3. If you can, have your prescription filled for 90 days. It will cost less than if you bough 30 days worth over 3 months. One of the reasons is that pharmacies charge a filling fee for each prescription filled. </p>
<p>4. Will you medication allow you to take a half-tab? For example, if you are taking lisinopril 20mg, you can purchase 40mg tablets and break them in half  a savings of 50%. Always check with your health care provider and/or pharmacist firstnot all medications can be broken. Also, consider those nifty little pill cuttersthey work wonderfully. </p>
<p>5. Do you really have to take all that medication in the first place? The best advice I can give you is to put everything into a bag and take it with you to your appointment. Make sure that you and your provider are on the same page when it comes to medications/supplements that you are taking. This is especially true when you are seeing more than one prescribing provider. You may have duplicates that you are unaware of. Always askdo I really need this?</p>
<p>6. Keep an up-to-date list of your medications with you at all times. </p>
<p>7. It is your responsibility to learn the names of your medications and why you are taking them! Most likely, your provider does not know what you mean by the little pink pill or the white capsule. There are literally dozens of each.</p>
<p>8. Check your medications before you leave the pharmacy. If something does not look right, ask about it right then. DO NOT take something you are unfamiliar with. </p>
<p>9. Follow label directions. If you are taking something for osteoporosis, chances are you were told to take it with a full glass of water, sitting upright while avoiding any other food or fluids for 30 minutes. They were not making it up! This goes for the labels on over-the-counter preparations. This is one time that if a little is good, a lot must be better can be a dangerous thought. </p>
<p>10. Another very important tip. If at all possible, it is in your best interest to have a primary care provider. Ideally, he or she will coordinate your care with specialists, and keep track of all the medications you are on. If you have a complete profile on record, then between your one primary provider, and your one pharmacist, any problems in your medications should be picked up. </p>
<p>11. This bonus tip comes from a lesson taught to me by a wonderful patient of mineIf you and your partner both take medication, do NOT mix them up. It can have serious consequences! </p>

Chest discomfort. Most heart attacks involve discomfort in the center of the chest that lasts more than a few minutes, or that goes away and comes back. It can feel like uncomfortable pressure, squeezing, fullness or pain.<br />

What Causes Heart Attack?<br />

Atherosclerosis is a gradual process in which plaques (collections) of cholesterol are deposited in the walls of arteries. Cholesterol plaques cause hardening of the arterial walls and narrowing of the inner channel (lumen) of the artery. Arteries that are narrowed by atherosclerosis cannot deliver enough blood to maintain normal function of the parts of the body they supply.<br />
Atherosclerosis and heart attack<br />
Occasionally the surface of a cholesterol plaque in a coronary artery may rupture, and a blood clot forms on the surface of the plaque. The clot blocks the flow of blood through the artery and results in a heart attack (see diagram below). The cause of rupture that leads to the formation of a clot is largely unknown, but contributing factors may include cigarette smoking or other nicotine exposure, elevated LDL cholesterol, elevated levels of blood catecholamines (adrenaline), high blood pressure, and other mechanical and biochemical forces.<br />
Many different medicines are used to treat and prevent heart attacks. Nitroglycein helps reduce chest pain. You may also receive strong medicines to relieve pain.<br />
Antiplatelet medicines help prevent clot formation. Aspirin is an antiplatelet drug. Another one is clopidogrel (Plavix).<br />

    Is Blood Pressure Treatment Color-Blind?<br />

    Because of this, the American Heart Association and other health groups recommend more aggressive blood pressure treatment for African-Americans than for other racial groups. The normal goal of treatment is to reduce blood pressure to below 140/90. But the goal for African-Americans is now 130/80.<br />
    In the newly published study, researchers analyzed data from a subgroup of participants in the Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial (ALLHAT). Their goal was to look at the risk of heart disease and death from heart disease in African-Americans with high blood pressure taking one of the three commonly prescribed medications.<br />
    When first published in 2002, ALLHAT challenged the pervasive thinking that newer blood pressure drugs worked better to treat high blood pressure and prevent some forms of heart disease than water pills, which have been used to treat high blood pressure for many decades. The ALLHAT study showed that a water pill worked better.<br />
    ALLHAT researcher Jackson T. Wright Jr., MD, PhD, says the trial has not affected clinical practice to the extent that he had hoped.<br />
    &#8220;The message has not really gotten out,&#8221; he says. &#8220;There has been a slight increase in diuretic use but not to the extent that one would anticipate from the data that are now available.&#8221;<br />
    Participants in the ALLHAT study, conducted between 1994 and 2002, took one of three drugs to lower their blood pressure, a leading cause of heart disease. They were given one of three commonly prescribed blood pressure medications: the diuretic Hygroton, the ACE inhibitor lisinopril, or the calcium-channel blocker Norvasc.</p>
